Activity Description

Aspergilloma and chronic cavitary pulmonary aspergillosis (CCPA) are the 2 forms of chronic pulmonary aspergillosis (CPA) that result from the inhalation of Aspergillus spores. Aspergilloma specifically occurs when Aspergillus species, most commonly A fumigatus, colonize preexisting lung cavities, often in individuals with underlying conditions such as tuberculosis, bronchiectasis, or emphysema. These conditions share overlapping clinical features, making diagnosis and management particularly challenging, with the unpredictable course and limited clinical data further complicating effective management. Diagnosing aspergilloma relies on characteristic radiographic findings, serological tests for Aspergillus-specific immunoglobulin-G antibodies, and fungal cultures from bronchoalveolar lavage. Although some aspergillomas regress spontaneously, others may lead to life-threatening hemoptysis. Treatment aims to stabilize symptoms, reduce hemoptysis risk, and prevent disease progression.

This activity reviews the epidemiology, diagnostic criteria, and evolving management strategies for aspergilloma and CCPA within the broader context of CPA, focusing on their global prevalence, especially in regions with high tuberculosis rates. This activity also highlights key predisposing factors such as bronchiectasis, emphysema, sarcoidosis, lung abscess, and lung cancer. In addition, this topic focuses on the chronic colonization of Aspergillus species in lung cavities or bronchiectatic parenchyma, commonly referred to aspergilloma, pulmonary mycetoma, or fungus ball. This activity provides clinicians with up-to-date knowledge and tools for evolving diagnostic and management strategies, thereby enhancing clinical decision-making and improving patient outcomes. Additionally, this activity highlights the collaborative roles of various healthcare professionals, such as infectious disease specialists, pulmonologists, radiologists, and surgeons, in optimizing patient care through an interdisciplinary approach.
